I never got back to investigate this properly. Having moved from pfSense to a Linux reverse proxy, I have confirmed that Outlook 2003 RPC over HTTP works with squid3 3.1.20 when squid is compiled with the enable-ssl option (edit debian/rules file).

It appears this solution does not allow Outlook RPC over HTTP to work. I am still investigating.

seems I'm too stupid for these simple instructions :) Squid always gives me an error page saying "cant forward your request". can anybody help me?
